Athletic (Sports) Physicals
The athletic (sports) physical assessment is required to be on file prior to try-outs for extracurricular or interscholastic sports in grades 7-12. The State of Illinois Certificate of Child Health Examination Form may be used for this purpose if it has been completed during the past twelve months. An IHSA Pre-participation Examination Form is the preferred form and is acceptable for thirteen months from the date of the examination. Either acceptable examination should remain current through the end date of the sport. Should either exam expire during the sport’s season, a new examination will be required to continue participation in the sport. If you are uncertain as to whether there is a current examination on file for your student, please contact your school’s health office at least two weeks prior to try-outs for confirmation of one being on record.
